<b><font size=+2>Visa Information</font></b>


<p>The citizens of <b><i><font color="#CC0000">Austria, Belgium, Bulgaria, Canada, Cyprus, Czech Republic,
Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Ireland, Italy, Japan, Korea,  Latvia, Liechtenstein, Lithuania, Luxemburg, Malta, Netherlands,
Norway, Poland, Portugal,
Romania, Slovakia, Slovenia, Spain, Sweden, Switzerland,
UK, USA </i></b></font> may enter Ukraine
<b>with valid passports</b> of their
respective countries for travel abroad  for a period of <b>up to 90 days</b> (It would make
sense to have an invitation letter).

<p>To enter Ukraine, <b>no visa</b> is required for persons who hold
an ordinary travel passports  of <b><i><font color="#CC0000">Armenia, Azerbaijan, Belarus, Georgia, Kazakhstan, Kyrgyz Stan,
Moldova, Russia, Tajikistan</font></i></b> or <b><i><font color="#CC0000">Uzbekistan</font></i></b> (It would make
sense to have an invitation letter).

<p>For citizens of&nbsp; <b><i><font color="#CC0000">Argentina, Croatia,
Mexico, Turkey</font>
</i></b>faxed
invitation from the Organizing Committee would be sufficient to apply to
the Ukrainian consulates in their countries in order to get Ukrainian visa.
Please request such invitation only when you are reasonably certain that
you are coming to the conference.
<p>Citizens of <b><i><font color="#CC0000">Brazil, Serbia, Republic of Montenegro,
Venezuela</font>
</i></b>need
an original invitation from the Organizing Committee and to apply to the
Ukrainian consulates in order to get Ukrainian visa. Please request such
invitation only when you are reasonably certain that you are coming to
the conference.
<p>Citizens of <b><i><font color="#CC0000">Australia</font></i></b> need a faxed invitation from the
Institute of Mathematics (signed by the Director and with the official
Institute seal) and to apply to the Ukrainian consulates in Australia
in order to get Ukrainian visa. Please request such invitation when you
are reasonably certain that you are coming to the conference.

<p>Citizens of other countries (in particular, <b><i><font color="#CC0000">Algeria, 
P.R.
China, Costa Rica, Egypt, India,
Iran,
Jordan,
Kuwait,
Morocco,
Oman, Pakistan, Republic of South Africa, Saudi Arabia, Tunisia, Zambia</font></i></b>) 
need an original special invitation
from the Organizing Committee and to apply to the Ukrainian consulate in
their respective countries<b><i> </i></b>in order to get Ukrainian visa. To arrange
such special invitation, we need the following additional information:

Citizenship.

2. Full name as in the passport.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
3. Date and place of birth.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
4. Home address.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
5. Work place and address.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
6. Current position.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
7. Passport No.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
8. Passport expiry date (with day and month).
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
9. Country and city where you plan to apply to the Ukrainian consulate
for obtaining visa.
<br>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
10. Scanned copy (.pdf or .jpg) of the first page of your passport, as
well as copies of pages with your latest two-three visas (if available).
<p>As soon as the invitation is ready we will mail the originals of the
invitations to you by regular mail.
<br>Please request such invitation only when you are reasonably certain
that you are coming to the conference.
<br><font color="#CC0000">Deadline for application for the special invitation is April  21, 2009.</font>
<p><font color="#CC0000"><b>WARNING:</b> </font>Visa cannot be obtained
in the airport upon arrival.
